A user task shows an embedded form the diagram does not contain

Why it matters
A user task that refers to an embedded form that is not in the diagram opens with no form.
How to fix it
Add the form to the diagram, or correct the reference.
Where you see it
The Problems tab lists it under Code. In a terminal, penstock code reports it with the file and line. This check is part of Penstock Pro, and Team in a pipeline.
