Connection is not drawn at right angles
The check drawing/diagonal-edge, in the group Drawing.
Why it matters
A BPMN diagram reads as horizontal and vertical lines; a slanted one stands out as a mistake.
How to fix it
Straighten the segment, or lay the diagram out again.
Where you see it
The Problems tab lists it under Readability, and outlines the element on the canvas. In a terminal, penstock check --only drawing reports it as DIAGONAL-EDGE. Auto layout often fixes it for the whole diagram at once.
