Connections run along the same line
The check drawing/merged-edge, in the group Drawing.
Why it matters
Two connections drawn on top of each other read as one, so a reader loses one of them.
How to fix it
Move one of the connections a few pixels aside, or lay the diagram out again.
Where you see it
The Problems tab lists it under Readability, and outlines the element on the canvas. In a terminal, penstock check --only drawing reports it as MERGED-EDGE. Auto layout often fixes it for the whole diagram at once.
