An option list repeats a value
The check form/repeated-option, in the group Forms.
Why it matters
Two choices with the same value submit the same thing, though the user can tell them apart.
How to fix it
Give every option its own value.
Where you see it
Press Check in the form editor. The Problems tab of the sidebar lists it. In a terminal, penstock check reports it as REPEATED-OPTION for every form. The form check is lint, so it is free.
